Long-Term Disability Details

  • An employee and/or employer paid, voluntary, long-term disability insurance plan.
  • The monthly disability benefit may not exceed 60% of covered monthly compensation.
  • Four elimination period plans are available. The elimination period determines when the accident and sickness benefits will start after a continuous disability – on the 31st, 61st, 91st or 181st day.
  • Accident benefits are paid up to age 65 or for five years, whichever is greater, but not beyond age 70.  Sickness benefits are paid for five years or to age 70, whichever occurs first.
  • There are no minimum participation requirements.
  • The monthly benefit is determined by the insured’s Covered Monthly Compensation, up to a maximum monthly disability benefit of $6,000.
  • The Covered Monthly Compensation equals an employee’s base compensation, exclusive of overtime, bonuses and other such compensation.

For groups of three or more, Voluntary Long-Term Disability requires no evidence of insurability for benefit amounts up to $1,200 when the employee enrolls during initial eligibility period.

In addition, an employee's Guaranteed Issue amount may increase to as much as $2,500 based on additional medical information and income.

Employees may choose higher monthly disability benefits (up to $6,000 – not to exceed 60% of Covered Monthly Compensation) subject to insurability.
