      Coverage Details

  • There is an additional cost per $1,000 of selected benefit amount.

  • Waiver of premium included.
  • When an employee reaches age 65, benefits are reduced to 65% of the original amount, and are further reduced to 50% of the original amount when an employee reaches age 70.
  • All benefits are paid in a lump sum.
  • Accelerated death benefit included.
  • Coverage ends at retirement or when the employee leaves the company.  However, coverage may be converted.  Employees have the option to continue coverage on an individual basis upon leaving the company.
